Name Change

Requirements:

  • You must file for a name change in the Circuit Court of the city or county that you reside in.
  • If you are Petitioning to change the name of your child, you will need consent from the other parent. If the other parent does not consent to the name change you may want to seek the help of an attorney.
  •  You must provide a copy of your birth certificate and all documentation showing any previous name changes (i.e. divorce decree, name change order, etc.)

Instructions for Adult Name Change:

  1. Fill out the Application (PDF) in front of a notary or deputy clerk.
  2. Fill out the Order (PDF).
  3. Provide a copy of your birth certificate and all other documentation showing prior name changes (if applicable).
  4. Filing Fee of $41.00 
  5. Name Changes can be submitted in person or by mail.
  6. Processing times vary, typically one to two weeks.
  7. After the Order is signed the Clerk's Office will mail you a certified copy, you will use this to make all other necessary changes.
