Requirements:

• Civil Cover Sheet (PDF)
• Petition for Restoration of Driving Privilege - Habitual Offender (PDF)
• A copy of your DMV transcript that is less than 30 days old
• A copy of your DMV Compliance Summary that is less than 30 days old
• A completed Travel Information packet
• Filing Fee ($110)

After filing your Petition you will need to contact RAASAP  at 540-645-6310 to set up an appointment. (This only applies if you have been convicted of a DUI.)

*Please note that it is the Petitioners duty to keep their travel information updated, if a Restricted License needs to be changed between Court dates there is a $10 fee.
